Life/AD&D Insurance      Employer Paid - Great West Life

MHCD provides each employee with a life insurance benefit equal to their annual salary ($100,000 maximum) at no cost to the employee.

Life Insurance      Voluntary - US Life

  • Each employee is able to purchase life insurance for themselves and their dependents through United States Life Insurance Company.
  • Each employee is eligible to purchase 5x their annual salary ($300,000 maximum) in life insurance for themselves and their spouse.
  • Employees may elect to purchase $2,500, $5,000, $7,500, or $10,000 for their dependent children.
  • All life insurance policies over $10,000 are medically underwritten.

  • MHCD provides, at no cost to the employee, a disability program (income protection).
  • The plan provides both short-term and long-term benefits.

Short-Term Disability (STD) Benefit Summary

  • 60% of your monthly salary with a $1,250 maximum monthly benefit
  • 30-day qualification period: you must be disabled for 30 days before you can receive a benefit
  • Maximum benefit period of nine 9 weeks after qualification period

Long-Term Disability (LTD)Benefit Summary

  • 60% of your monthly salary with a $5,000 maximum monthly benefit
  • 90-day qualification period: you must be disabled for 90 days before you can receive a benefit
  • Coverage to age 65
  • 3-month survivor benefit
