Intensive Case Management Services

These programs provide case management services to persons with severe and persistent mental illness primarily in the community.  The teams are multi-disciplinary, psychiatrists, nurses, medical assistants, social workers, licensed clinicians and case managers working together to assist MHCD consumers in their recovery process.  Each treatment plan is individualized predicated on what the consumer desires.  Services provided routinely address symptom management, housing, finances, legal issues, employment and education, medical and dental care, substance use, family life, and other basic needs and meaningful life activities. Students will work individually with MHCD consumers, as group co-facilitators, in crisis intervention situations, and as team members. MHCD intensive case management teams provide treatment using the Evidence Based Practices of Assertive Community Treatment (ACT,)   Integrated Dual Diagnosis Treatment (IDDT,) Dialectical Behavior Therapy (DBT,) and Trauma Recovery and Empowerment Model (TREM.)

Program Description: El Centro de las Familias is a multi-cultural clinic which provides culturally competent outpatient mental health care to the Latino/Hispanic community and to the South West community in Denver. The clinic strives to provide culturally competent, Spanish/bilingual recovery-oriented services to children, adults and families by focusing on individual strengths and natural community resources.  With an emphasis on Spanish speaking and bilingual consumers, the El Centro staff focuses on actively engaging consumers to use the strengths of their culture, values, and existing resources for optimum recovery.
